Deliberative Democracy Engagement Process
Deliberative Democracy is an inclusive process to engage community or large group members as part of a new initiative, change or development that will affect them. This video is part of a larger program explaining the development of Mayo Clinic’s biobank. The Deliberative Democracy process was used as a foundational effort in that case and will also be used as part of the Southeast Minnesota Beacon Program.
Local Public Health HIE Initiatives
Daniel Jensen, Associate Director of Olmsted County Public Health, provides an overview of the public health work being done in SE MN to build the Health Information Exchange foundation and then expand upon that infrastructure to develop advanced transition of care exchange processes. These include: school nurse portal, Clinical Data Repository, case manager notification at point of client hospital admission or ED visit, ED in-reach social worker case finding, and medication reconciliation.
Parsing Medication Lists from CDA Documents
Consuming and parsing medications from electronic documents received from other institutions can assist community based providers such as local public health agencies or home care agencies in better understanding which medications to expect to find while conducting a medication reconciliation with their clients. Furthermore, transferring these consumed medications into the local medical record will speed the process and likely improve accuracy during the medication reconciliation allowing the healthcare professional to focus more upon the needs of the client. This video describes an approach piloted by Olmsted County Public Health in PH-Doc working within the SE MN Beacon Community of Practice. The focus of this Beacon Community of Practice is to improve patient outcomes through the utilization of healthcare technology in an environment of strong collaboration utilizing best practices.
Improving Hospitalization Notification to Stakeholders
Notifying community case managers/ care coordinators of their clients’ hospitalization at point of admit can allow better client centered discharge planning to occur. Utilizing healthcare technology to identify and facilitate this notification is a practical application of these tools. This video describes one such intervention pilot by Olmsted County Public Health working within the SE MN Beacon Community of Practice. The focus of this Beacon Community of Practice is to improve patient outcomes through the utilization of healthcare technology in an environment of strong collaboration utilizing best practices.
